Membership Application Temporary Instructions

UPDATE

Membership Applications for 2020-2021 now available

We are now accepting applications for the 2020-2021 School Year! Renew now before our fees go up on June 1st. This is our first membership fee adjustment and is necessary to keep up with our costs.
We encourage you to start your application process soon to guarantee your spot. Co-Op Classes fill up fast, so act now.
We have made temporary arrangements for how we receive these applications during these times, so follow the steps below.

1. First complete the JOIN form here.
2. Then, under the MEMBERSHIP tab, download all 3 forms (Youth – one per child, Adult and Confidential Screening). Fill these out and mail them back to us with notarized signatures and a copy of your drivers license.
Mail forms to: 13835 Moonstone Canyon Dr., Riverview, FL. 33579.
3. Send payment via paypal or include a check in the envelope with the forms (checks payable to Bell Shoals Baptist Church).
Please remember if you are using PayPal to complete the following steps: In the “Add a Note” section, please list your name and what you are paying for (membership, co-op fees, Background checks, etc).
Secondly, please click the box to pay friends/family. This keeps us from having to add a convenience fee.
4. To join Co-Op Classes, fill out this form.
